My cousin was born without a womb and with a sealed canal. Medically speaking, she was intersex. But that never stopped her from spending nearly every night out with a different man on her arm, flaunting her supposed conquests like trophies pinned to her chest.

One day she leaned in close, a conspiratorial grin curling her mouth, and whispered, "You know, Francesca, there's more than one way to make a man happy."

I tried to talk sense into her. Warned her about the infections, the risks, the sheer recklessness of it. Honestly, she was begging for a disease. She only laughed it off.

Then she set her sights on a made man's son. The Armetta bloodline, the kind of name that came with an estate behind iron gates and enough soldiers to make a city hold its breath. Suddenly she wasn't so cocky. She came to me asking whether she should have surgery to "fix" herself, to make everything work the way a man of that rank would expect on the night the two houses were bound.

I warned her. Surgery carries risks. A woman's body isn't some instrument built to please a man. If she wanted to stand beside someone, she had to put her own health and dignity first.

But she wasn't interested in caution. She believed her tricks in bed, or whatever version of them she could manage, would be enough to keep him leashed.

On the night the alliance was sealed, no matter what she pulled from her twisted little playbook, he turned on her all the same. Said she disgusted him.

Word moved through the Families like wildfire. The Armettas cast her out like yesterday's garbage. The pact was broken before the vows had cooled. Her dream of marrying into a ruling bloodline? Dead in the water.

She lost everything. Her pride, her place at the table of people who mattered, the alliance itself.

Her dream of marrying into a ruling bloodline? Dead in the water.

And then she blamed me for all of it.

She poured gasoline over me and set me alight.

"You bitch! If you hadn't talked me out of the surgery, I wouldn't be a laughingstock in front of every Family in this city!"

When I opened my eyes again, I was back in that exam room. The very day she told me her secret.

"Seriously, Francesca, it's not the old country anymore," she giggled, twisting her waist like she was auditioning for something. "You really gotta stop thinking like a nun."

"There are so many ways I can make a man happy. Way more than just that one." She licked her lips, slow and deliberate.

"Last night my man and I went at it again. He was so wild. We went through eight rubbers. Strawberry."

She actually looked proud. Beaming. Glowing, as if she'd won something worth winning.

Meanwhile I sat there, trembling. Every word out of her mouth landed like a knife between my ribs, dredging up memories from the life I'd already lived. And died for.

I drew a long, shaky breath and forced myself calm. My hands clenched into fists beneath the table, but I smiled.

"Impressive," I said flatly.

She mistook the sarcasm for praise and lit up brighter. "Of course I'm impressive."

Then she leaned in, that soft smile sharpening into something else. "Anyway. You've been with Nico for what, two years? And you still haven't done it." She clicked her tongue. "Aren't you afraid someone else swoops in and takes him while you sit here playing the Virgin Mary?"

I didn't answer.

"Men don't care whether you're pure. What they care about is whether you can please them. Keep them happy and they never leave."

Of course she dragged Nico into it.

The "man" she claimed to have spent that wild night with? That was Nico Barone. My man.

In my last life, I didn't learn until after I was dead that she'd been seducing him behind my back. The two of them were already tangled in each other long before I understood it, right up to the night the marriage between us was meant to happen.

And even after she burned me alive, Nico still stood at her side. He put his name to a statement begging the court to go easy on her.

Watching her play at innocence now, wearing that twisted pride like jewelry, I nearly crushed my own fists under the table.

"Nico respects my choices," I said tightly. "That's why I love him."

For just a second something flickered across her face. Envy. It vanished fast, buried under the usual smirk.

"I don't get you," she scoffed. "You're perfectly healthy, and you treat your body like some priceless relic behind glass. What a waste."

She leaned closer, her voice like honey laced with poison. "But hey. If you're so set on staying pure, maybe take a few lessons from me."

I peeled off my gloves and nodded toward her half-buttoned blouse. "Aren't you worried your mother finds out?"

That wiped the smirk clean off her face. Her fingers froze mid-button. Her eyes narrowed, and I watched them fill, quick and practiced, while her other hand pressed into her own forearm hard enough to leave marks.

"She won't find out," she said sharply. "Not unless someone tells her. And you're not stupid enough to bite the hand that feeds you. Are you, Cousin?"

Back when both my parents were killed in a car crash during my studies, it was Donna Ashford who stepped in and covered everything. She made certain I finished my training and became what the Family needed.

But that generosity came with a debt.

Donna Ashford was no saint. She'd been the kept mistress of a powerful capo once, betting that giving him a son would lock her a seat among people who mattered. Life played a cruel joke on her instead. She gave birth to Alessia. Intersex, sterile, useless to every fantasy she'd built of climbing up through the bloodlines.

So she took the hush money, raised Alessia alone, and never let go of the dream of trading her daughter for a name that meant something.

When I became a physician, the kind who works below the surface where no legitimate clinic can ever know, she saw her opening.

She wanted me to operate. Clear the canal. Stitch in a false proof of purity. Make Alessia look like a blushing, untouched bride so some ruling son would never know the difference. And because the aftercare would go on and on, the hormones, the maintenance, the quiet visits, I'd be the perfect built-in solution kept close to the household.

What she didn't know was that her daughter had already been tearing herself apart.

Of all the wild things my cousin had bragged about in that back-room clinic where I patched up soldiers who couldn't be seen at any legitimate hospital, two of her "tricks" left me horrified.

As the Family's made doctor, I knew the dangers better than anyone. We're not just talking about the kind of infection you can't explain to the Doc twicethis stuff could tear muscle, leave long-term damage, even cause internal scarring. When I warned her about the risks, she just laughed at me like I was some convent girl.

"Relax. We used protection," she said with a roll of her eyes, like that magically made it safe.

Not long after she and Nico got tangled up, Donna Ashford introduced her to some heir she'd been grooming her foran alliance she'd been clawing toward for years, a chance for the Ricci household to marry into a ruling bloodline.

He had one requirement. His pledged bride had to be a "pure" woman. Untouched. Worthy of the name he'd give her.

Donna Ashford practically knelt to me for the procedure. She wanted me to "restore" Alessia. Build the illusion of an untouched bride. I tried to explain the risksloss of sensation, scarring, complications the Doc himself couldn't fixbut she didn't care. She saw legitimacy within reach, and I was the instrument to seize it.

I pulled Alessia aside and told her the truth. Every detail.

Surprisingly, she backed off. She was convinced her seduction alone could land the heir. Said men of the ruling Families didn't care about natural births anywayhalf of them arranged for others to carry the bloodline. This way, she could keep her figure and still bind herself into the name. A win for everyone.

Or so she thought.

Turns out, the alliance night didn't go as planned. No matter how she tried to please him, the man went perfectly still, and his voice dropped to almost nothing. The moment he understood the fraud, he called off the pactright there, in the marriage bed. In front of two bloodlines.

Humiliated. Rejected. Dishonored before an entire dynasty.

And somehow, she decided I was the one to blame.

That night, she lit the fire that killed me.

Thinking about it now, I knew what I had to do.

If I couldn't stop her, I'd at least make sure she and Nico were bound together. Let the faithless associate and the scheming climber wreck each other, too busy with their own ruin to burn anyone else's life down.

"Don't worry, Alessia," I said sweetly. "Your secret's safe with me. Donna Ashford will never hear it from me. Omert."

Once the door clicked shut, I pulled out my phone and texted my darling man.

[Miss you, babe.]

Along with it, I sent a few choice photos. Lingerie, props, the whole nine yards.

Nico took the bait instantly, replying with drooling emojis like some starving stray. I didn't answer. I let him stew.

By the time I finished my shift patching up a soldier's split knuckles, I had dozens of unread messages waiting. Nico was clearly on fire, thinking I'd finally changed my mind.

That night, I went all incopying every flirtatious line I could find and sending them one by one, building the heat until he was begging.

[Babe, can we video chat? Please?]

His voice was thick with lust.

I turned him down with a laugh, claiming I had to shower.

Not long after, I heard Alessia's bedroom door click open. She slipped out into the dark of the household like a girl sneaking to a hookup.

She came back at dawn, legs wobbling.

Over the next few days, I checked Nico's online cart.

Sure enough, he'd ordered a stack of toys. Expensive ones.

Every time he wanted her, he leaned on the excuse of being "out of town on Family business." This latest "trip" had stretched a whole monthand my cousin's gait had gotten noticeably strange. She was taking forever in the bathroom, too.

Donna Ashford finally sensed something was off. She pounded on the bathroom door, straightening the string of costume pearls at her throat, shrieking through the wood.

But Alessia brushed her off with a casual, "Just an upset stomach."

Then one day, she showed up at the clinic, pale as a ghost.

"S-something's wrong," she muttered.

I had her lie down on the exam table and did a quick check.

The results weren't good. Her muscles were showing early fatigue and loss of tone. Classic wear and tear. Nothing was holding up well.

"You and your man really need to stop. Or at least ease off, or it's going to cost you"

Before I could finish, she snapped.

Her fingers dug into her own forearm, hard, leaving pale crescents behind. "You're just jealous because I actually know how to enjoy life, while you're trapped in your miserable little cage!"

She wasn't going to let go of Nico. That much was clear. She never could stand the idea of me being happy. Over the years, any time a man showed the slightest interest in me, she'd swoop in like a hawk, flaunting herself until he forgot I existed. It wasn't about love. Not for her. It was about winning. Always proving she was somehow above me.

I gave her a sweet little smile. "Suit yourself. I'll prescribe you something for the pain."

She left with a tube of medicated ointment, and sure enough, after a few applications she said things felt much better. Which only made her bolder. She doubled down, slipping out more often, chasing that high like it was oxygen.

But the party didn't last.

Donna Ashford eventually caught on. Maybe it was the way Alessia kept vanishing in the dead of night. Maybe it was the excuses stacking up thin. Either way, one night the game was up. Donna Ashford laid down the law and put her on lockdown, guarded like a debt she couldn't afford to lose.

With his playmate under house arrest, Nico had no choice but to come crawling back to me.

He texted that he was "back from Family business."

What a shame.

I replied with a breezy, "Oh no, what a coincidence! I just took a job out of the citya beachside retreat this time!"

And every single day on that beach, I kept the game going. Flirty messages. Double-meaning photos. And just for fun, I signed him up on every mailing list and dating site I could find.

Just as I expected, once Alessia was locked away behind Donna Ashford's doors, Nico came crawling right back to me. Only this time, he wasn't looking for love. He was looking for release.

He spiraled fast. Late-night rooms above the wrong social clubs, sketchy hookups with women no soldier would claim, reckless spending he couldn't cover on an associate's cut. His order history became a parade of increasingly bizarre toys. I saved every image, every receipt, filing it away the way a made doctor files a case no hospital can ever see. Ammunition, quietly stockpiled.

Then I broke up with him.

No drama, no long messages. I just told him it was over. At first, Nico kept showing up at the clinic, lingering near the service entrance where the Family's men came to be patched without questions, trying to talk to me, trying to win me back. But when his attempt result for nothing, he gave up.

Alessia, of course, was thrilled when word reached her. Thought she'd "won." Thought her irresistible charm had finally done the trick.

And Nico? He wasn't just licking his wounds. He was hell-bent on payback. The two of them dove headfirst into something even more reckless than before. The toys got darker. The stunts got more unhinged.

It wasn't long before the consequences caught up.

She started smelling off. Like something rotting underneath all that imported perfume. And instead of fixing the problem, she just doused herself in more of it, trying to mask it, trying to pretend nothing was wrong.

Then came the big day.

Donna Ashford had found her next target. A rich heir named Lorenzo Armetta, the Underboss-in-waiting of the Armetta Family. She'd pulled out all the stops. A gown that cost more than most soldiers earned in a year, fine jewels, the salon booked before dawn. The whole nine yards.

"Alessia, this is your chance," she gushed while the tailor knelt at the hem, pins between his lips. "The Armettas are old blood. Lorenzo was raised on honor and money both. And he likes innocent, wholesome girls. Girls like you."

I caught the way Alessia flinched. Just barely, but it was there. The panic. The guilt. Her nails found the soft skin of her own forearm and pressed in.

"I already have a man," she muttered, brows drawn tight.

"Then be rid of him," Donna Ashford snapped. "You think some low associate can hold a candle to an Armetta?"

She waved it off like it was nothing. "Besides, you're not even sleeping with that one. Break it off, have Francesca do the restoration, and you'll be good as new. Clean. Pure enough to pledge between two bloodlines."

That was the plan. One quiet surgery away from being a virgin again. One lie away from marrying into a ruling Family.

But for once, Alessia hesitated. She faked a stomach ache and refused to go, no matter what Donna Ashford said, her fingers leaving small crescent marks in her arm the whole time.

Donna Ashford wasn't about to let a golden alliance slip through her hands. So instead, she turned to me. Her rings flashed as she straightened the strand of pearls at her throat, adjusting them the way she always did when a plan started slipping.

"You'll go in her place," she said, like it was a simple favor. Like I owed her the blood in my veins.

I refused, of course.

That's when she pulled out her favorite line. The one she'd used for years, the one that always made my blood run cold.

"Don't forget who gave you everything you have, Francesca. I took you in when no one else would. I fed you, raised you, kept a roof over your head. And this is how you repay me? With defiance?"

I ended up in Alessia's gown, seated beside her at a sit-down dressed up as an introduction, in the ballroom of one of the Family's own hotels. Pretending to be her.

In my past life, I hadn't paid much attention to Lorenzo Armetta. All I remembered was that he was a man ruled by principle in a world that had none. But meeting him again this time, there was a strange sense of fate in the air.

He was handsome, well-mannered, and refreshingly grounded. Not at all like the spoiled heirs who thought their fathers' names made them untouchable. Like me, he'd only ever given himself once before, and he carried that quiet integrity like a man who understood exactly what it cost.

When I told him I believed in a lifelong, exclusive kind of love, he lit up. Said women had thrown themselves at the Armetta name for years, but all he really wanted was someone who shared his ideals. His "one and only."

That night, we talked for hours. Everything just clicked.

We met a few more times after that, on neutral ground, quiet places where no one watched too closely. He seemed to like me more each time. Soon enough, Donna Ashford's doorstep was buried in fine things sent in the Armetta name.

Then Alessia saw him. Sharp suit, leaning against the Cullinan, the kind of stillness that made other men lower their voices when he entered a room. Her entire attitude flipped. That very night, she cut Nico loose and called me up, claiming Lorenzo had always been meant for her.

But I wasn't about to roll over this time. I met her words with a smile full of barbs.

"Poor Alessia. Are you addicted to stealing my men?"

She scoffed.

"I let you have Nico because he was trash. But Lorenzo? Lorenzo has standards. And you don't meet a single one of them."

Her mask slipped just for a second, then she smoothed it back into place, her voice thick with smug confidence.

"Think you're so great?" she hissed. "Believe it or not, Lorenzo Armetta is going to be mine. And when that day comes, I'll savor the moment you finally understand what you are. Nothing but a failure. A woman who could never keep a man."

Rage boiled up inside me and I slammed my glass down so hard the rim cracked.

I slammed my glass down so hard it cracked.

Donna Ashford came storming in, and when she heard Alessia had finally "come to her senses," she was thrilled.

"Well," she said, straightening the good silver on the sideboard with quick, fussing fingers. "Looks like Alessia's finally come to her senses."

Then she turned to me and dropped the hammer, her voice going frosty and flat with command.

"Francesca, let's not forget you were only standing in for Alessia at that sit-down to begin with. It's only right you hand Lorenzo back to her."

My hands shook. My jaw was clenched so tightly I could feel my molars grinding. I couldn't believe I was hearing this.

And just when I thought she couldn't cut deeper, she delivered the line she knew would paralyze me.

My hands trembled. My jaw clenched so hard I thought my teeth might break. I couldn't believe what I was hearing.

And then she hit me with the line she knew would shut me up.

"Don't forget who gave you the life you have, Francesca. Don't you dare repay kindness with betrayal."

The words lodged in my throat like a shard of glass. My vision blurred as tears poured down, silent and relentless. The room had gone very still, the way a room does when everyone knows something is being taken and no one will stop it.

If Mom and Dad were still here, they would've never let this happen. They wouldn't have just stood there and watched me get humiliated like this.

No. I wasn't going to crumble. I still had Lorenzo. He said he liked me. He promised he wanted to spend forever with me.

Lorenzo would stand by me. He had to. He wasn't the kind of man to be led by the nose by Donna Ashford's schemes. I ran out of the house, my heart pounding as I tried to call him. But the line was busy. I tried again. And again. Nothing but that damn busy tone, humming in my ear like something already going wrong.

Two hours later, my phone finally rang.

"Lorenzo Armetta," the screen read.

I snatched it up, hopeful. "Hello?"

"Francesca, I have something to tell you." His voice was flat, quiet in that way that should have warned me. "I don't think we're a good match. Let's break it off."

Just like that. A single sentence hit me like a gut punch. It echoed in my ears like thunder.

Yesterday, when we parted at the gate of his family's estate, he had held me so tight, whispered against my hair that he couldn't wait to make me his wife, to bind our names together in front of both bloodlines.

And now this?

"Why?" I choked out, biting my lip so hard I tasted blood.

There was a long pause. Somewhere on his end, a clock was ticking, too loud in the silence.

Then, before Lorenzo could say anything, I heard another voice.

"Do you really need someone to spell it out for you?"

It was Donna Ashford. "Lorenzo met your cousin and realized she's the better fit for a Family like his," she said, her voice smug and cruel, the voice of a woman fingering jewelry she hadn't earned. "She's prettier. More refined. She plays piano, she dances, she knows how to stand beside a man of his rank. And what do you have? You're the girl who patches up bleeding soldiers in a back room, elbow-deep in blood and other people's filth. Lorenzo's being generous not to mention the risk of catching something off a woman like you."

Every word was a slap to the face.

I had never been ashamed of what I did. I kept men alive. I did the work no legitimate doctor could ever know about. Lorenzo wasn't this shallow. He couldn't be.

"Shut up!" I snapped, my voice trembling. "I want to hear it from him."

"She's right, Francesca." His voice had gone even quieter now, almost nothing, and I should have understood what that stillness meant in a man like him. "I'm the only heir of the Armetta Family. The woman I marry has to carry herself with grace. She has to know how to hold a room, how to stand at my side in front of the other Families. Your cousin is more polished. More charming. If I have to choose between the two of you, of course I take the best."

He paused.

"But hey. At least we never did it. Don't hate me for this, okay, Francesca? Who knows. We might still end up famiglia one day."

Screw that. Lorenzo and his whole bloodline could burn for all I cared. I hung up, floored the gas pedal, and let the road blur beneath my tears.

I made up my mind right there. I was going to move out, find a place near the clinic, and never owe Donna Ashford anything again. Whatever debt she claimed hung over my head, she could consider it paid in full.

When I pulled into the lot behind the clinic, I sat a moment, breathing through it. Then I opened my photo album. One by one, I deleted everything tied to Lorenzo. Every picture.

Every message. Then I looked up, and saw something that stunned me into silence.

All my heartbreak, all my bitterness, it vanished in an instant. Because right in front of me, I saw an opening. If they all wanted to play dirty, fine. I slowly rolled down my sleeves and buttoned the cuffs, one and then the other, the way I did before a procedure. I'd play right back.
